Fixing a Complex Knee in a Tiny Patient

Geumdong’s Comeback Story: Fixing a Complex Knee in a Tiny Patient 🐾

Geumdong, a lively young Maltese, came to us for persistent limping on his hind leg. What initially appeared to be a routine issue turned out to be more complex—he was diagnosed with medial patellar luxation along with an ACL (cranial cruciate ligament) injury, creating significant instability in his knee.

To address this, we performed a patellar luxation repair using trochleoplasty combined with Tibial Tuberosity Transposition (TTT).This approach deepens the groove for the kneecap and repositions the tibial attachment to improve alignment—restoring more normal biomechanics to the joint.

Because of the concurrent ACL instability, additional stabilization was incorporated to support the knee during healing.

The surgery went smoothly, and Geumdong began his recovery.

But like many young dogs, Geumdong had his own ideas.

Despite instructions, he managed to chew out his sutures, causing the incision to partially reopen. Not long after, he returned—limping again.

On examination, we found mild inflammation and superficial incision dehiscence, but fortunately, there were no signs of deep infection or surgical failure. His limping was most consistent with discomfort from the irritated incision rather than a problem with the internal repair.

We carefully cleaned and prepared the area, administered local anesthesia, and re-closed the incision—this time using skin staples for added strength.

Geumdong handled the procedure like a pro.

With strict E-collar use, activity restriction, and close monitoring, his recovery is now back on track. His case is a great reminder:

👉 Even the most advanced orthopedic surgery—like trochleoplasty + TTT for patellar luxation with ACL stabilization—depends heavily on proper post-operative care at home.

Preventing licking and limiting activity are just as critical as the surgery itself.

Geumdong is now on his way to a full recovery—and hopefully staying out of trouble this time.
